JOB SUMMARY: The position is responsible for providing personal
care and designated health related services designed to maintain
the client’s physical and emotional well-being. Catholic Eldercare
is committed to recognition and effective management of each
resident’s well-being. Every employee (within the scope of their
position) is responsible to seek relief for any resident observed
to be in pain (physical, spiritual or otherwise) through
therapeutic intervention (hand holding, physical touch,
acknowledgement of discomfort) and reporting the discomfort to the
Direct Care: assist the resident with personal cares which may
include all or some of the following:
Redirection of a confused client.
May assist with supervision, bathes and dresses residents,
provides personal care in toileting, combing hair, mouth and nail
care, and provide lotion rubs, taking vital signs, weights and
intake and output as instructed by the charge nurse.
Assists as needed with their meals, providing between meal
nourishment. Pass water and snacks.
Assist residents in transfers and walking, in a manner
consistent with established safety procedures and following all
requirements utilizing transfer equipment.
Transports residents within the nursing home, insuring that the
transport is performed safely, promptly and in a manner consistent
with established safety procedure.
Maintains clean or orderly physical environment for residents.
Make beds, clean and straighten rooms and equipment daily as
required, and maintains a sanitary environment for residents at all
Assist in the use of adaptive devices such as hearing aids and
Answers devices for signaling nursing and nursing assistants
(using call lights or communicators) and deliver messages as
Assist each client to reach and maintain the highest level of
independence, allowing for choices that promote safety and
well-being and foster self-help. Maintains an awareness of the
physical and emotional well-being of assigned residents and
promptly report a changing status to the nurse. Reports all types
of unusual occurrences immediately.
Assist with treatments as delegated by the Nurse per
individualized care plan after competency training and testing.
Observe client and monitor and report changes in physical and
emotional condition such as change in attitude, loss of appetite,
loss of endurance, increase in pain and weight.
Demonstrates sensitivity to residents needs and maintain a
respectful manner. Uses patience and tact when communicating with

Ability to read, write and comprehend English and simple
instructions, correspondence and documentation through verbal
directions and computerized charting.
Must be able to demonstrate proficiency, accuracy and competency
in aide given to residents.
