Job Title: Home Health Speech-Language Pathologist; PRN
Supervised by: Patient Care Services Director Job Summary:
Evaluates, plans and administers comprehensive speech therapy
modalities/regimens as prescribed by a physician to patients with
speech, voice, language, cognitive-communicative and
oral-pharyngeal swallowing disorders in their place of residence
for the purpose of improving and/or restoring speech and/or
swallowing functions. Demonstrates Competency in the Following
Areas: Performs accurate and comprehensive speech, language and
swallowing assessments. Obtains physician orders for certification
and recertification of home speech therapy services. Ensures that
changes in patients' clinical status are communicated to other
healthcare team members and HHA office staff verbally and in
writing in a timely manner. Ensures that physician orders are
communicated to other healthcare team members and HHA staff in a
timely, accurate and thorough manner. Coordinates care planning and
delivery with the physician and other healthcare team members and
HHA staff to facilitate optimal patient outcomes. Delivers speech
therapy modalities appropriate to the patient's clinical status,
needs and age. Able to appropriately assess and reassess a
patient's pain. Utilizes appropriate pain management modalities
that include, but are not limited to, physical therapy modalities.
Educates the patient and/or family in effective pain management
modalities. Provides speech therapy modalities in an environment
that optimizes patient safety and reduces the likelihood of
medical/health care errors. Supports and maintains a culture of
safety and quality. Addresses and supports cultural practices
providing such practices do not harm the patient or others, and do
not interfere with the planned course of speech therapy.
Participates in the HHA's information management and performance
improvement programs. Complies with the HHA's policies. Accurately
completes OASIS data set. Adheres to all applicable laws,
regulations and standards. Completes and submits necessary
documentation in accordance with HHA policy and applicable
regulations. Submits verification of completed visits in accordance
with HHA policy. Participates in interdisciplinary team conferences
in accordance with HHA policy. Maintains current knowledge of
community resources. Identifies and accesses appropriate external
and internal resources to meet patient and/or family needs and to
facilitate optimal patient outcomes while home care services are
being provided, and for transfer and discharge processes.
Demonstrates effective time management and organizational skills;
provides patient visits as scheduled; completes patient care within
the designated time. Articulates HHA grievance/concerns procedure.
Articulates HHA adverse event/incident reporting procedure. Assures
continued competency by maintaining and updating professional
performance, knowledge and skills. Professional Requirements:
Adheres to HHA dress code; appearance is neat and clean. Adheres to
legal practice standards, including applicable federal, state, and
local regulations and standards. Completes annual education
requirements. Maintains patient confidentiality at all times. Wears
identification while providing patient care. Attends annual review
and in-services, as scheduled. Attends 12 staff meetings annually
or reads and returns all staff meeting minutes. Maintains currency
of professional knowledge by participating in continuing education
offerings. Participates in the organization's performance
improvement and continuous quality improvement (CQI) activities.
Practices in a manner consistent with the professional and HHA code
of ethics. Represents the organization in a positive and
professional manner. Complies with all organizational policies
regarding ethical business practices. Communicates the mission,
ethics and goals of the organization. Requirements Regulatory
Requirements: • Master's or Doctoral Graduate in the area of Speech
Therapy or Speech Pathology from an accredited university or
college. • Certificate of Clinical Competence in Speech-Language
Pathology awarded by the American Speech Language and Hearing
Association. • Current valid state Speech-Language Pathologist or •
In the case of an individual who furnishes services in a state
which does not license speech-language pathologists: • Has
successfully completed 350 clock hours of supervised clinical
practicum (or is in the process of accumulating surpervised
clinical experience) • Performed not less than nine (9) months of
supervised full-time speech-language pathology services after
maintaining a Master's or Doctoral degree in speech-language
pathology or a related field; and • Successfully completed a
national examination in speech-language pathology approved by the
Secretary. • One (1) year of work experience. • Prior home health
experience preferred. Valid state driver's license and reliable
automobile. • Proof of current automobile insurance. • Current
health certificate/physical examination and TB testing. • Current
BCLS certification. • Current liability insurance. Language Skills:
• Able to communicate effectively in English, both verbally and in
writing. • Additional languages preferred. Skills: • Understands
regulations/standards applicable to home healthcare. • Knowledge of
general speech therapy modalities and scope of speech therapy
practice within the state of Arkansas. • Flexible, yet organized,
with ability to exercise sound judgment. Physical Demands: • For
