Additional Job Description The RN Care Manager II - IPCM, as part
of the interdisciplinary team, evaluates, plans, advocates, and
coordinates care from admission to discharge, ensuring a secure
transition post-hospitalization. This role includes assessing
patients' health requirements, creating tailored care plans, and
coordinating services to maximize health outcomes. Effective
communication with patients, families, and healthcare providers is
vital. Additionally, this position is key in advocating for
patients' needs, promoting health education, and facilitating
seamless transitions of care across various healthcare settings.
Clinical expertise, critical thinking, and compassion are essential
for enhancing patient well-being and improving healthcare outcomes.

Qualifications: Education: Bachelor's Degree in Nursing is
required. Licensure: A current RN License in the state of
employment is required. Experience: At least 1 year of experience
in an acute care hospital setting is required. Required
Certifications: BLS (Basic Life Support) certification is required
at the time of hire or within 90 days (approximately 3 months) of
employment. Preferred Certification: Certification in Case
