Under the guidance of the primary care operations team, the Primary
Care Patient Care LPN collaborates closely with our Primary Care
Providers to ensure safe, timely, effective, efficient, equitable,
and patient-centered care. This role involves partnering with
patients, healthcare providers, and other clinical team members to
enhance quality outcomes for all individuals. Responsibilities will
be executed in alignment with our mission, values, and service

of an accredited practical nursing program. Licensure: Must hold a
valid LPN (Licensed Practical Nurse) license in the state of
practice. Preferred Experience: At least 6 months of clinical
healthcare experience, ideally in a primary care setting.
Additional Knowledge, Skills, and Abilities: Ability to work
effectively within a team and collaborate with all care team
members. Strong written and verbal communication skills. Customer
service-focused. Excellent organizational and interpersonal
abilities. Adaptable and positive demeanor. Meticulous attention to
detail and accuracy. Self-motivated and capable of performing tasks
independently within the scope of practice with minimal
supervision. Physical Requirements: Must be able to push, pull,
and/or lift up to 50 pounds regularly. Requires prolonged periods
of standing and walking during each shift. Ability to grip, reach,
bend, kneel, twist, and squat to fulfill job duties. We Provide
