The Care Manager plays a vital role within the interdisciplinary
team, responsible for assessing, planning, advocating, and
coordinating patient care from the moment of admission through to
discharge. This position ensures a smooth transition following
hospitalization. Responsibilities are carried out in alignment with

Summary: The RN Care Manager II - IPCM is an integral member of the
interdisciplinary team, tasked with assessing patient health needs,
creating personalized care plans, and coordinating services to
enhance health outcomes. Effective communication with patients,
families, and healthcare professionals is crucial in this role.
This position also involves advocating for patients’ needs,
promoting health education, and ensuring seamless transitions of
care across various healthcare environments. Clinical expertise,
critical thinking, and compassion are essential for fostering
patient well-being and improving healthcare results.
Responsibilities are executed in accordance with our organization's
mission, values, and service standards. Qualifications: Education:
Bachelor’s Degree in Nursing is required. Licensure: A current RN
License in the state of employment is mandatory. Experience: At
least 1 year of experience in an acute care hospital setting is
required. Required Certifications: BLS (Basic Life Support)
certification must be obtained at the time of hire or within 90
days (approximately 3 months) of employment. Preferred
Certification: Certification in Case Management is advantageous.
